CAREER CENTER

The Social Work Program Coordinator is responsible for all aspects of Advance Care Planning via Group Visits within the facility. They manage the daily operation of the program, develop policies and procedures for program operation and prepare reports and statistics for facility, VISN, and national use. The incumbent is based in the medical center and/or facility in order to support real time, in person, response to referrals, training, and education. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
