Learn more at www.multicarejobs.orgPosition Summary:The Crisis Care Coordinator is responsible for providing follow up services for individuals seen in crisis and helping coordinate referrals and warm hand offs to appropriate services in the community to help stabilize and/or mitigate future crisis The incumbent provides direct assistance in gaining access to services; coordinates care within and across systems; ensures linkage to appropriate services; conducts assessment of the eligible recipient to determine service needs; eval
