The Licensed Clinical Social Worker is responsible for the development, implementation and evaluation of case management services and counseling, and facilitation of support groups for clients with health related issues. The licensed Clinical Social Worker reports to the Director of Behavioral Health
Conducts comprehensive intake interviews including medical and psychosocial assessment and development of treatment plans for clients in assigned service areas
Provides counseling and case management services for individuals and families including clients with substance abuse and mental health problems
Monitors client needs and service provision and conducts follow-up as needed
Develops and maintains linkages with other health and social service agencies
Develops and facilitates support groups for men, women, teens, parents and/or school-aged children
Conducts and/or participates in assigned family unity meetings and workshops
Provides client advocacy activities including coordination of services for medical and dental care, emotional and practical support, public assistance, community services, and mental health services
Maintains client documentation in compliance with agency protocols
Participates in case conference activities to coordinate care for clients
Prepares written reports as assigned by supervisor
Assists with the preparation of reports to funded sources as needed
