Community Health Workers promote access to services, provide health education and assist community members in navigating the health care and social services systems. This Community Health Worker will specifically engage with Behavioral Health patients - children/teens,adult and substance use disorder patients
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Attends training program provided by Christiana Care and the Penn Medicine Center for Community Health Workers.
Develops and delivers health education and training programs for our patients and the community.
Creates connections between vulnerable populations, the health care system and community/state resources.
Assists patients in the community and/or clinic setting.
Travels to patient's homes to discuss Community Health programs and to assist in setting up services.
Responsible for patient chart reviews and referring patients to other programs as appropriate.
Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
Facilitates healthcare and social service system navigation.
Establishes and maintains professional relationships with community leaders, faith leaders and others responsible for providing services to the community.
Participates in community events promoting the Office of Health Equity and Community Health, as needed.
Educates staff, health providers and stakeholders about community needs.
Provides culturally appropriate health education on topics related to chronic disease prevention, social and behavioral health needs, etc.
Advocates for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between clients and institutions.
Collects data as required by supervisor.
Maintains professional competence by participating in local and national training sessions and conferences, as offered.
Works collaboratively and effectively within a team.
Establishes positive, supportive relationships with participants and provides feedback to other members of the team.
Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors, and office staff, from diverse cultural and socio-economic backgrounds.
Participates in regular program and department meetings.
Performs assigned work safely, adhering to established departmental safety rules and practices; report to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Perform other related duties as assigned and required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
High school diploma required, bachelor's degree preferred.
Experience with children/adults that have high behavior and mental health needs a must.
One-year community and patient experience required, more preferred.
Public speaking, group facilitation and excellent writing skills preferred.
Computer experience required.
Demonstrated ability to work with diverse populations required.
Bi-lingual language skills are a plus.
