This Senior Social Worker position is located at the Edward Hines Jr. VA Hospital in Hines, IL and is part of the VA's Great Lakes Health Care System in the Veteran's Integrated Service Network (VISN 12). The Transplant Senior Social Worker performs specialized treatment of Veterans both being assessed for Renal Transplant and those who receive a Transplant at the Edward Hines Jr. VA Hospital. Incumbent incorporates multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Major Duties: Independently complete comprehensive transplant evaluations to identify psychosocial contraindications to transplant. Provide case management and care coordination to facilitate appropriate delivery of health care services, incorporating complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Link the Veteran with services, resources, and opportunities, in order to maximize the Veteran's independence, health, and well-being. Independently formulate and implement a treatment plan including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial needs. Conduct timely assessment of all at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. Provide discharge planning and ongoing case management to Transplanted Veterans in Acute Care at Hines VAMC. Assists Program Coordinator with clinical coverage for unexpected social worker absences related to patient needs during the absences. Provides feedback to Service Chief and Supervisor and keeps supervisor informed about the program's progress in meeting relevant performance goals. Ensures compliance with program standards. Serves on committees, work groups, and task forces at the facility and VISN level or in the community as assigned by Supervisor.
