This (list position) position is located in Social Work Service at the VA Bedford Healthcare System location. This position is a full-time at 40 hours per week. The incumbent provides comprehensive clinical support to Veterans and their families in the Neurocognitive Outpatient Clinic. Interviews Veterans and family members / significant others to determine Veteran's clinical needs, presenting problems and the Reviews all data, subjective and objective and makes sound clinical assessments, identifying strengths and needs in the context of the family / support system.ir causes and the impact of such problems on the Veteran's functioning and health as part of a comprehensive psychosocial assessment Assesses risk factors, acuity and need for services Develops treatment plans in coordination with team members and is charged with a specialized Social Work practice as a key component of treatment Develops and maintains a professional standard of treatment and initiates methods to promote efficient practice performs multiple duties in support of Dementia Research and Studies through Geriatric Service communicates status of patient illness and special needs of families to inpatient staff at the VA or in the community to facilitate a smooth transition mentors other social workers and supervision to Social Work trainees.
