The Social Worker position is located in the Behavioral Health Service line whose duties and responsibilities center on providing clinical and case management services to Veterans being seen at the Behavioral Health Interdisciplinary Program (BHIP) level of care at the Pittsburgh VA Medical Center. The incumbent provides clinical psychosocial, case management, and counseling services at an independent practice level to eligible veterans, couples, and families to meet biological, psychological, social, and environmental needs. Veterans treated come from a broad range of cultural and military experiences. Their heath care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem-solving. Services provided in outpatient setting include, but are not limited to: Independently evaluates, identifies and screens veteran patients for psychosocial needs. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Coordinates community-based services, to include other VA programs and community agency programs. Identifies high risk patients and provides case management services. Establishes and maintains effective therapeutic relationships with veterans and their families. Independently provides consultation services to other staff regarding psychosocial needs of veterans.
