This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The incumbent provides foundational support to the implementation of the Whole Health (WH) program, ongoing training (VA-specific programs TEACH for Success and Motivational Interviewing), and consulting to PACT clinical staff in support of PACT staff developing and refining effective health behavior coaching skills. Works with Health Promotion Disease Prevention Program Manager (HPDP PM) and the MOVE Coordinator in support of Whole Health coaches and volunteers. Works with the Health Promotion and Disease Prevention (HPDP) Program Manager and HPDP Committee members to develop and promote facility-wide HPDP programs. The incumbent may also provide support to the Tobacco Use Cessation and MOVE (weight loss) programs as needed and appropriate. Major duties include, but are not limited to: Plans, develops, adapts, implements/oversees and assesses efficacy of health behavior interventions for the promotion of Whole Health for personalized, proactive and patient driven care that engages the Veteran to the highest level of health and well-being (e.g., weight loss and maintenance, tobacco use cessation, promoting patient self-management interventions targeting issues such as increasing participation in regular exercise, healthy eating, sleep hygiene) in close collaboration with the Health Promotion/Disease Prevention Manager and others. Contributes to training of primary care team and others in evidence-based methodologies to effectively communicate with, motivate, coach, and support patients in being aware of relevant health risks, assisting patients in clarifying personal goals for health promotion and disease prevention programs, and developing individualized patient self-management plans. Works with Primary Care and Nursing leadership to provide specific training, guidance, and mentoring of Primary Care team members to facilitate effective coaching (i.e., healthy lifestyle coaching to support patient self-management).Serves as or supports the lead clinician in tobacco use cessation programs. Collaborates/partners with Primary Care Behavioral Health staff with respect to implementation of health coaching skills within PACTs across VAPAHCS facilities. Provides consultation and training to Primary Care and Mental Health Service Line staff in the enhancing, maintaining, and supporting health behavior program for the promotion of health and prevention of disease and enhance patient self-management of chronic disease (e.g., diabetes, HIV, Hepatitis, COPD) to prevent secondary complications and improve long-term outcomes and enhance quality of life. Develops programs together with other Health Promotion/Disease Prevention, Tobacco Use Cessation, and Patient Health Education staff members for teaching and/or improving the PCP's core behavioral health skills. The Health Behavior Coordinator/Health Psychologist will have a primary clinical oversight role in health behavior coaching and is likely to oversee or share oversight of health coaches/ training specialist. The incumbent will carry out appropriate consultation with discipline-specific leaders.
