The VA Palo Alto Health Care System is recruiting for a term not to exceed 18 months Social Worker (Program Coordinator) for the Social Work Service Line. The Social Worker (Program Coordinator serves as program coordinator of the Intimate Partner Violence Assistance Program and is administratively responsible for a clinical program providing treatment to Veterans. Incumbent manages the daily operations of the program and provides direct client care services in the program area. Incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget and various control points assigned to the program within the assigned areas. The incumbent manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. Incumbent plans, organizes and directs administrative, operational, and Veteran activities for the program. Major duties include, but are not limited to: Participates in direct Veteran, family, Caregiver, and VA employee contacts to identify and assess risk, safety, supports and the development and application of directed clinical treatment interventions. Advises and collaborates with interdisciplinary teams throughout the medical center on intimate partner violence (IPV) issues. Creates educational tools, develops programs and implements training focused on specific IPV needs/issues. Provides education and in-service training to VAMC employees, Veterans, families and Caregivers. Provides ongoing consultation and support to colleagues regarding IPV assistance. Develops and facilitates support groups (face-to-face or VANTS telephone line) and educational programs. Distributes educational materials provided by VA, local, state and national IPV organizations. Conducts Veteran, family, caregiver and VA employee interventions to facilitate the identification of intimate partner violence, psychiatric illness, emotional distress and social needs as treatment issues. Provides referrals to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. Collaborate with the Patient Aligned Care Team (PACT), Mental Health and Geriatric Primary Care clinicians to provide Veteran and staff education on intimate partner violence. Develop a library of IPV assistance information material for distribution through the various inpatient/outpatient and specialty clinic settings. Work Schedule: Monday through Friday 8:00 AM to 4:30 PM Financial Disclosure Report: Not required Telework, if approved, would be on a regularly scheduled basis, but this is not a virtual or remote position. This position will remain open until 9/30/2021 or until filled whichever comes first. Applicants who apply before the midnight of 4/7/2021 will be referred first.
