The Social Worker (Program Coordinator) serves as the Advanced Care Planning via Group Visit (ACP-GV) Coordinator and develops and implements the ACP-GV Program and ensures compliance and collaboration with local facility and VA Central Office Advanced Care Planning via Group Visit Program initiatives. This position will be based at the Houston VA Medical Center but may be required to travel to outlying CBOCs that are approximately 50 to 150 miles from the MEDVAMC as part of official duties.
