Our exemplary team of Inpatient Social Workers are essential liaisons between patient, family, physicians, community, and organizational resources to promote the coordination of a comprehensive treatment plan.
Our Inpatient Social Workers collaborate with physicians, staff nurses, and RN Care Coordinators to determine patient care needs, readiness for discharge, coordinate transitions of care to rehabilitation facilities, or other community based resources. This individual will play a significant role in making connections and referrals to the health agencies, rehabilitation facilities, and other social and governmental departments that would be the best resource in ensuring patient needs are met.

Engaging in timely and efficient discharge planning
Collaborates closely with patient, family, physicians, RN Care Coordinators, therapists, and other members of the multidisciplinary team
Screens new admissions that meet high risk criteria
Completes comprehensive psychosocial assessments and social determinants of health
Builds and maintains working knowledge of in house, community, county, and state resources
Strong verbal and written communication skills
Master's Degree in Social Work Required.
American Heart Association "BLS" Provider Required within 6 months of hire
KNOWLEDGE, SKILLS, & ABILITIES:
Two years' experience in health care field, familiarity with medical terminology preferred
Experience in dealing with extended care facilities and community healthcare agencies preferred
Working knowledge of insurance coverage, various governmental programs, available community care agencies and long-term care facilities preferred
Pennsylvania Social Worker Licensure preferred
Case Management Certification (CCM) preferred
