Responsible for the overall coordination of interdisciplinary services provided to patients throughout their admission for rehabilitation. Screens potential admissions for medical appropriateness at CHRISTUS Santa Rosa facilities as well as other community sites. Ensures the provision of total case management including admission, orientation, evaluation, treatment plan development, patient care conferences, specialty team conferences, discharge planning, community re-entry and follow-up. Actively participates in and provides leadership for specialty program development.
Degree in Rehabilitation field to include: MSW; BSN/CRRN; LPC; Clinical Psychologist; or Certified Insurance Rehab Specialist.
Three (3) years clinical experience in rehabilitation preferred.
One (1) year experience in case management/program management is desirable.
Licensed in the area of the individual's Clinical expertise, as appropriate.
