Job Purpose 

Under general direction, the Licensed Clinical Social Worker provides counseling, psychotherapy, consulting, and outreach services for patients of special populations or with health care needs.  Assists in planning and implementing patient services and program components of integration of behavioral health and primary care.  Performs other administrative duties as requested.   Bilingual skills in English/Spanish are strongly desired.

Duties and Responsibilities 

• Provides assessment, short-term treatment, and crisis intervention and case management to client caseload and provides clients with consulting and additional referral to other community resources.
• Completes an assessment of patient/family needs with respect to physical, functional, psycho-social, environmental, financial, educational and other needs.
• Monitors the care plan to ensure the effectiveness and appropriateness of services; ensures that services are being delivered and meet the needs of the patient.
• Advises and makes recommendations to physicians, patients and families regarding the availability of resources and coverage, locates the resources, and arranges for the provision of cost effective services for patients.
• Maintains a database of patients served that includes all support services and efforts made to resolve issues of care.  Manage Behavioral Health Care Planning dashboard in EHR.
• Prepares and regularly maintains progress notes, closing, and/or transfer summaries on patients and related duties as required.
• Demonstrates satisfactory knowledge and clinical judgment to provide appropriate care for clinic patients.
• Fosters a multi-disciplinary environment, coordinating with primary care providers and psychiatrist as needed, in the format of a primary care behavioral model.
• Coordinates or follows-up on health education resources.
• Protects the confidentiality of medical records of various media forms (including computer generated, audio, video, or other forms of media) as per Georgia Highlands’ policies and HIPAA regulations.
• Attends and participates in in-services/continuing education as needed.
• Maintains confidentiality of Center business and quality improvement activities.
• Participates in improving organizational performance activities as requested.  Responsible for monitoring the reporting requirements for Behavioral Health.
• Participates in peer review, quality assurance, provider meetings, and other clinical meetings.
• Assists in updating protocols and principles of the practice as requested.
• The LCSW adheres to all GHMS Clinical Policies and Procedures that are reviewed and approved by the Board of Directors, i.e. locking charts; evidence based standards, etc. and complies with Practice Standards and Practice Certification requirements (PCMH, etc.)
• Adheres to productivity standards and meets at least the minimum requirements.
• The LCSW follows Corporate, Ethical, and HIPAA policies and procedures to ensure the confidentiality of all patient information.
• The LCSW performs additional duties as requested and appropriate that are needed to ensure the ongoing success of the organization.
