Directs the delivery of social services for the geriatric psych program. Responsible for providing therapy services to patients and their families as a part of the multidisciplinary team with most emphasis on group therapy completing Psychosocial Assessments, facilitating family therapy and individual therapy as ordered, developing, implementing and reviewing treatment plans and case management discharge planning.
Master's Degree from accredited school of social work. Employee must be licensed on the Illinois Department of Financial and Professional Regulation website as a Licensed Clinical Professional Counselor or be a Licensed as a Social Worker by the State of Illinois. Minimum of two years previous experience in health institutions, child welfare agencies, psychiatric clinics or family counseling services preferred. One-on-one care training required.
