HRI Hospital is seeking a Full-Time Clinical Coordinator for the Partial Hospitalization Programs. This is a Monday through Friday salaried position. Hours of work are 10am to 6pm.
The HRI Hospital Clinical Intake Coordinator is responsible for working collaboratively with the Director or assigned designee from the PHP/IOP Programs. This individual will receive referral requests from clinicians, patients, family members, managed care company staff, and others. The Clinical Intake Coordinator will also be responsible for assessing appropriateness for PHP/IOP level of care and coordinating access to transportation services for clients identified as in need. Intake Coordinators must answer incoming calls; receive faxed referrals, process demographic, clinical and insurance information in a manner consistent with service excellence. Clinical Intake Coordinators ensure efficient and effective call processing and assessment, leading to a patient admission to a PHP/IOP program or providing appropriate information to caller.Â
Receiving referral information from caller or faxed referrals and assessing appropriateness for PHP/IOP/Sober Housing.
Collaboratively discussing with Program Director or designee client needs and program placement once accepted.
Completing an intake call sheet and gathering all necessary demographic, clinical and financial information on call sheet.Â Ensures information is accurate and complete as call sheet is transmitted to hospital clinical and business office staff.
Ensures verification of insurance eligibility and benefits including primary and secondary payors.
Completing Intake Biopsychosocial Assessments for clients admitting to the PHP/IOP/Sober Houses.
On occasion, completing in-person screenings with potential Sober House clients.
In a timely manner, notifies the Program Director of impending Sober House Referrals .Follows up in a timely manner to assure information received and updates on status from PHP/IOP staff.
Schedules PHP/IOP intake appointments by completing PHP intake call sheets and scheduling appointments
Sending completed call sheets and insurance verification/authorization to the Program Director, Lead clinicians, Evaluation Services, and Business office.Â Â
Maintaining accurate, up-to-date Sober House bed availability including updating theÂ Program Director ofÂ pending admissions or cancellations
Contacting the Program Director when a review is necessary or patient delayed.
Updating external bed board on regular basis.
Administering follow-up calls to clients who have discharged from program after 30, 60 and 90 days to discuss client experience in treatment.
Adhering to department procedures related to waitlisted patients.
Â Assist in training new staff as requested.
