Assist with the program's assessment process; monitoring referrals and linkages to other human services identified through the assessments
Review care plans and assessments generated by the Health Coaches, Meals-on Wheels assessors, and interns
Supervise a small team of health coaches who conduct home visits across multiple counties for individuals of all ages with complex health needs. Additionally help bring in interns which will also be supervised by this position.
Communicate frequently with LIFE program staff and volunteers to identify those clients in need of additional services and/or follow-up
Maintain the confidentiality of agency procedures, results and information about participants, and clients in conformance with HIPAA principles
Provide occasional informal presentations to local health care facilities and share information on LIFE’s programs with other service providers
Excellent computer skills – Word, Excel, Access and e-mail
Minimum one year's experience working with an elderly population.
Demonstrated ability to work in a multicultural and multidisciplinary team environment.
Ability to work with minimal supervision.
Excellent oral and verbal communication and meeting facilitation skills.
Excellent organizational, interpersonal, and communication skills
Ability to treat each client, volunteer, and staff member with dignity, respect, and professionalism
Ability to determine and spot risk factors regarding clients provide follow up -usually by phone- as necessary
Flexibility, patience, and a high responsiveness to client and other stakeholder needs
Valid California driver’s license and current vehicle insurance
Current Licensed Clinical Social Worker credentials
Clear criminal background check
Must demonstrate strong written and verbal proficiency in the English language. Additional proficiency in Cantonese, Mandarin, or Spanish is a plus

We provide Home and Community Based services to older adults in need: Home-delivered therapeutic meals, door-through-door destination assistance, Fall Prevention, and other programs.
